This report examines the life of a young child called “Jake,” who was in care from the age of five months until his death just prior to his second birthday. This report includes a review of the services Jake and his family received from the Ministry of Social Services (MSS) and Saskatoon Health Region to determine if they received the services to which they were entitled.
