Mandatory Review into Child Deaths

This report reviews the circumstances of nine young people who passed away during the period of October 1, 2020 to March 31, 2021. Three were receiving services as children in need of intervention at the time of their deaths, and six within the previous two years, meeting the criteria for mandatory reviews. This report includes two young people who died prior to this time, but whose reviews had been stayed until this period. Two additional young people died during this period; however, their reviews have been stayed and will be released at a later date.
