This is a review of the Serious Occurrence Reports (SOR) filed to the Ontario Government when incidents occur in children's residences that are deemed " serious". These are mandatory reports and must be filed within 24 hours. The Provincial Advocate has undertaken this review to make recommendations for improve the quality of these reports and to and their effectiveness as safeguards to children's well-being.
