This recommendation and implementation report follows the case of an Labrador infant who died of traumatic injury while in his parents custody despite the family's involvement in community-based child protection services for concerns related to violence in the home. The report a lack of inter-service communication, collaboration, assessment and intervention and follow up, as well as documentation deficiencies. The Advocate reports that many of these same concerns have been raised in previous reports by the Advocate and "A Stolen Life" discusses implementation of those previous recommendations.
