Paediatric Death Review Committee – Medical and Child and Youth Death Review and Analysis: Report for 2019‐2021

The purpose of child and youth death reviews is to explore the circumstances related to the child or youth's death in order to honour their lives, learn from them, and make recommendations that may enhance the overall well‐being of young people and contribute to the prevention of further deaths. The Child and Youth Death Review and Analysis (CYDRA) Unit within Ontario’s Office of the Chief Coroner has been developing and transforming the processes for child and youth death reviews, replacing the former Paediatric Death Review Committee – Child Welfare (PDRC‐CW) with several distinct death review processes. The PDRC‐CW was a standing committee specific to the child welfare sector, limited to reviewing the deaths of young people involved with the child welfare system within the 12 months preceding their death.
